TRAINING QUIZ
Here's an example of a quiz that will help you verify that your training program is working. This quiz was adapted from a hazard communications quiz prepared by North Carolina State University. There's also an answer key at the bottom of the page that you can use to check your workers' answers (be sure to remove the key before you give them the quiz). As with any model form, you'll need to make revisions depending on your industry and the kind of operations you perform.

Here's a Model Form you can use to prove your company verified that workers understood the training you provided. Update the form each time you test your workers' health and safety knowledge and keep it in their training or personnel file. As with any model form, you'll need to adapt the form to reflect your methods of verification.
