MODEL NOTICE — OSHA VIOLATION CHALLENGE
Here's a letter illustrating how to write a proper notice of contest to challenge an OSHA violation. Remember, you have only 15 working days from the date you got the citation notice to file your notice of contest.

Dear Area Director:
Please be advised that ABC Manufacturing Company intends to contest the violation citations and penalties proposed for Items 3 and 4 of the OSHA Citation and Proposed Penalty #00000, dated August 2, 2005 and referenced above.
A copy of the citation, which was received by this office via certified mail on August 1, 2005, is attached to this notice.
