Here's a Model Implementation Plan for a JHSC based on the case study hypothetical:

ACTIONS	REASONS	WHO IS RESPONSIBLE?	DEADLINES
Speak to building maintenance about shovelling the snow more frequently.	Eliminate the hazard of snow build-up that employees track into the builing.	John	Complete five days after meeting. Report results to special meeting in seven days.
Speak to builing services about installing a carpet inside the west entrance when it snows.	A carpet will cover up the slippery floor; it is best to take it up when there is no snow so that it dries.	Mary	Complete five days after meeting. Report results to special meeting in seven days.
Speak to building services and graphic design about making a sign to advise employees of slippery conditions when it snows.	A sign will remind employees of the danger and will be a back-up in case the carpet is not in place after a snowstorm.	Pat	Complete five days after meeting. Report results to special meeting in seven days.
