You touched upon 'arc flash' in your last paragraph. At my previous employer, we taught all the Maintenance emps to stand to the side of 'boxes' when opening them 'just in case'. Arc flash seemed to be one of those esoteric concepts that were extremely unlikely. Indeed I think many of use feel an arc flash will only occur with shoddy electical work. However in this case a new box with new components in a clean, dry electrical room 'blew'. We ended up with a Recordable due to the chest burns incurred but it could have been far, far worse. Thereafter we bit the bullet & sought proposals for a thorough arc flash analysis. The 5 digit cost is quickly seen as a bargain if you save one set of eyes or a life!
